1. General Adoption Petition – This form is the petition to request the number/age/sex of the children you want to adopt, as well as some general description. (health, complexion, character, etc.
2. Homestudy (performed by a licensed agency or social worker) – 2 authenticated copies are required.
3. Homesutdy Licensing - 2 authenticated copies. If your homestudy is performed by an independent social worker, this is just a notarized copy of your social worker’s license. If your homestudy is performed by an agency, this is a notarized copy of the agency’s license, along with a notarized social worker employment letter.
4. Notarized copy of the USCIS form I-171H – You keep the original, Use the following coversheet.
5. Passport copies, 3 sets, each set must be notarized with the following coversheet.
6. Marriage certificate, 2 certified copies, the apostille must be from the secretary of state, from the state the marriage took place.
7. Income verification – A letter written on letterhead by your employer, H.R., or accountant that states one’s position, salary, and monthly income for the last six months. This is needed for each adoptive parent. For one-income families, this form can be used to notarize the employment letter and a non-working parent can prepare a personal statement.
8. Last year’s W-2 – Notarized copy for the first 2 pages with the following coversheet.
9. Residence verification – SDA now requires two forms of residence verification via property rent or ownership. Families who own their house or apartment need to provide a copy of their deed - the shortest version, excluding any appendices, but listing owners' names and property address. A certified copy of the deed issued by the county clerk's office can be apostilled directly like a marriage license; alternatively, a regular copy of the deed should first be notarized using a coversheet and then apostilled.
The second form for home owners is this form, which
must be prepared on the tax assessor’s letterhead.
Families who are renting should obtain a copy of their rental agreement and staple it to the bottom of this form printed on the rental company’s letterhead whenever possible.
10. Medical Clearance - Your primary doctor can fill out the form completely, which needs to be copied on clinic/hospital letterhead, or may at the very least be stamped with the doctor’s clinic information. Proper diagnosis can be healthy or no abnormalities and negative or not found for tests. For patient ID numbers, doctors can put any unique reference numbers, such as social security number, insurance billing number, etc.

Registration Obligations - Residents of Alabama, Alaska, Arkansas, Delaware, Washington DC, Florida, Georgia, Kentucky, Louisiana, Maryland, Mississippi, Missouri, North Carolina, Ohio, Oklahoma, Oregon, South Carolina, Tennessee, Texas, Virginia, West Virginia fill out this form and register their children with the Consular Office of the Embassy of Ukraine in Washington DC.

Francisco.
Residents of Illinois, Indiana, Iowa, Kansas, Michigan,
Minnesota, Nebraska, North Dakota, South Dakota, Wisconsin fill out this form
and register their children with the Consulate General of Ukraine in Chicago.
Residents of Connecticut, Maine, Massachusetts, New
Hampshire, New Jersey, New York, Rhode Island, Pennsylvania, Vermont fill out this form and
register their children with the Consulate General of Ukraine in New York.
13. Interpol Police Clearance Request- 2 authenticated copies, The SDA will submit this form on behalf of the families.
I4. Information Disclosure Consent- This petition allows Ukrainian adoption authority to use famiy’s personal information in order to register them as prospective adoptive candidates.
15. Power of Attorney – The Power of Attorney form allows your facilitator in Ukraine to translate and submit your dossier to SDA on your behalf. Your adoption agency will provide a power of attorney for a specific facilitator.
16. Appointment Request Letter – This is a letter in which you ask the SDA for an appointment in a certain month, explain your reasons, specify which child you are looking to adopt and mention your power of attorney. The letter would have a blank space for the dossier registration number that would be filled in later on by the SDA. Please remember to sign the letter in blue ink and notarize and apostille it along with the other documents. Your adoption agency will provide a sample letter for you use as a template.
